To request an event appearance by Supervisor Lee or a representative from the District 3 Office, please fill out the following form. Due to the high number of requests received by our office, we encourage you to submit your request as far in advance as possible - most event confirmations require three weeks notice of invitation please. Please note that we respect and observe local health orders and safety protocol to ensure the well-being of all participants.
